Modern engines depend on that timing being accurate to well under a degree, so internal wear that would once have been tolerated now shows up as smoke, noise and failed emissions checks. Look for extended cranking, especially hot, poor throttle response, smoke across the rev range, fluctuating idle speed, and a gradual rise in consumption that cannot be explained by duty cycle.
